New patient visits need to be scheduled in advance by calling 801-302-0301. Please allow an hour for your first visit which includes a review of your medical history, exam, evaluation, and treatment.
The initial paperwork may take about 10 minutes to complete. If you prefer to download the forms and complete them at home simply click on the links below.
Remember to bring the following to your first visit:
- Current medication list
- Any imaging you’d like Dr. Hall to review
- Insurance card and driver’s license
- Form of payment (no personal checks please)
What you can expect your first visit
We provide treatment along with the exam at the first visit. Please check in at the front desk and have your insurance card ready to scan. You will have time to complete the intake forms, while the staff checks your insurance coverage. Verification of benefits is not a guarantee of coverage. It is recommended you contact your insurance before you come in.
To start, we will collect your vitals (height, weight, and blood pressure) and then you will see Dr. Hall. He will talk with you about your health history, your current condition and concerns and then perform a non-invasive neuro-ortho exam. This may involve some testing of muscle tone, flexibility, range of motion, strength, balance, and sensory response. He will then recommend treatment options, and depending on the severity of your condition or pain he will adjust you at that visit.
Patients say that Dr. Hall is a good listener and takes the time to explain things to them. Through the different phases of care he educates and instructs patients on how to resolve pain, avoid future episodes, and prevent injury. After the patient has a reduction in pain and reaches phase two, exercise instruction and home care is discussed. Condition specific exercises done regularly play a big role in strengthening and toning the area, and preventing future injury.
