FREE INSURANCE VERIFICATION

We may still be an in-network provider for some carriers not listed.  If your insurance is not listed, please call us for insurance verification.  You simply give us your insurance I.D. number, name birth date, and we can verify your insurance coverage, for FREE, in a matter of minutes.

SECONDARY INSURANCE

Often times your primary insurance will forward on the claim information to your secondary if they have it on file.  This saves a lot of time and insures expedited payment to your provider.  However, it is the patient’s responsibility to coordinate these benefits with the primary insurance carrier so that they have all the correct information.  Also, if your secondary insurance has a high deductible ($300-400) or will pay less than $7 it is not worth the effort to bill.

The patient is responsible to pay all co-pays, co-insurance and deductibles at the time of service.  We accept cash or credit, but we do NOT accept personal checks.
