How to Become a New Patient

(If you have had an AUTO accident or WORK injury please go to the Auto/Work Injury tab for instructions.  You will have additional paperwork to fill out.)

1. CALL FOR APPOINTMENT: 801-302-0301

   Call us today to set up your initial consultation and exam.  If you are using your insurance you will pay your co-pay, co-insurance, or portion of your deductible.  We will need to verify your coverage to determine the amount.  If you are paying cash and not billing any insurance you may receive a discount for paying at the time of service.  The total cost for the exam and care for the first visit is just $45.  (Laser Therapy, Spinal Decompression Therapy, and x-rays are additional cost.)

2. INSURANCE

   If you see that we are a provider for your insurance in the INSURANCE TAB, it still would be a good idea to have us verify your coverage before you arrive.  Give Cheryl your information over the phone and we’ll have it ready for you at your appointment.

3. NEW PATIENT REGISTRATION PAPERWORK

   You may fill out this paperwork in the office when you arrive, or print the forms online, fill them out, and bring them with you when you come.  We’ll need all four papers filled out and signed for EACH PATIENT.

   • New Patient Registration Form

   • Consent to Care
   • HIPPA Privacy Policy (read only)
   • HIPPA Consent Form (sign after reading HIPPA privacy policy)

4. LOCATION

   Here is a link to a map.  We are easily located on the corner of 104th South and Bangerter Highway in South Jordan, in the Copper Ridge Office Park, Building 3630, Suite 102.

5. POLICIES

   We ask that if you choose to bring small children they be on their best behavior.  We do have a “kids room” that has some toys and a movie playing.  We ask that they either stay in the kid’s room, or with you.  No eating or drinking or pets in the office.  Please arrive a few minutes early so we can keep on schedule, and be sure to bring your insurance card, ID, and form of payment.  Wear comfortable, modest clothing and plan at least an hour for your first visit.
